Exclusive VIP Aesthetic Membership Program

The Contour Club Rules & Restrictions

•  Pay $199 per month and receive ONE complimentary service from the membership service menu at no charge.
• Choose to Add-On One Additional service from the Add-On menu and pay the additional fee.
• Add-On Services can only be added to the coordinating complimentary service listed in the menu.
• Trade-in your complimentary service on one VIP Service from the menu and pay the upgrade charge.
• No other services outside The Contour Club Membership Service Menu can be chosen as part of the program.  No discounts or trade-in options can be applied to other services.
• All member services are performed at Sculpted Contours and there is no cash value to the membership program or its fees outside our office.  
• Minimum Initial Term 6 months.  No cancellations or terminations during the initial term.
• Cancellation after the initial 6 month term must be in writing and delivered to Sculpted Contours with a minimum 30 days notice of cancellation.
• A valid credit card must be kept on file and monthly membership fees will be billed and charged each month on the renewal date.
• You may roll-over one unused service to the following month.  Complimentary services not used within 60 days of the billing date will expire and be forfeited.
• Membership services cannot be combined with any other promotional offer or discount.
• Membership services are non-transferrable and non-refundable and can only be used by the member enrolled in the program.
• Some services require a consultation before booking.  No service will be offered or performed that are medically contraindicated regardless of membership.  Please confirm with our office if you are a candidate or excluded from any service listed in the membership menu. 
• Some services have required or recommended skin care product regimens to go along with them.  The products are NOT included in the membership fee or any add-on or upgrade fees.  Products can be purchased at the 10% discount provided in the program. 
• After the initial term has ended, monthly membership will continue on a month to month basis until cancelled by the member with 30 days minimum written notice.  Monthly billing will continue.  
• Extenuating circumstances including extended illness, military deployment or relocation more than 50 miles away from our office can be used to suspend temporarily the membership or terminate membership within the initial term with 30 days written notice and documentation of the cause  delivered to Sculpted Contours.
• Sculpted Contours reserves the right to change pricing, terms or service menu offerings that are part of The Contour Club. All members will receive minimum of 30 days notice of any alterations to the membership program with the option to cancel their membership per cancellation policy.  
• If a member is within their Initial Period, the terms of the membership in effect at the beginning of the membership will be honored by Sculpted Contours.  No change in price or decreased level of services will be implemented against any member in the Initial Period of 6 months.  At the end of the Initial Period, the revised membership fees and service menu will go into effect.  The member has the right to cancel their membership at the end of the Initial Period if they do not agree with the new changes with a 30 day written notice.
• If the service menu is expanded during the Initial Period, the member has the right to all expanded services immediately.  
• A Contour Club Membership Program contract will be signed by each person wishing to participate in the program.
